I understand that participating in a softball league involves inherent risks, including injury from physical activity, contact with others, equipment, and playing conditions. I voluntarily assume all such risks.

I certify that I am physically fit and have no medical condition that would prevent me from safely participating in softball activities. I agree to follow all league rules, regulations, and instructions provided by league officials.

In consideration of participation, I release and hold harmless the league organizers, sponsors, facility owners, and staff from any liability for injury, loss, or damage arising from my participation.

I understand that the league is not responsible for lost, stolen, or damaged personal property.

I grant permission for the use of my name, likeness, and photographs or video taken during league activities for promotional and marketing purposes without compensation.

I agree that this waiver is intended to be as broad and inclusive as permitted by applicable law. If any portion is found to be invalid, the remaining provisions shall continue in full force and effect.

I understand that equipment used in play, including bats and balls sanctioned by NSA, may vary in performance and may contribute to increased velocity and risk of injury.

I acknowledge that I have read and understand this waiver and agree to its terms.
